In therapy, the journey toward healing and growth is a collaborative process that relies on consistent effort and shared commitment. Regular attendance in psychotherapy plays a pivotal role in achieving meaningful progress. This article explores the importance of attendance, explains why maintaining a minimum 75% attendance rate is vital, and offers compassionate guidance for addressing barriers that might arise.

Why 75% Attendance Is a Meaningful Benchmark

     

Research consistently highlights the connection between regular attendance and positive therapeutic outcomes. Setting a benchmark of attending at least 75% of sessions helps ensure clients experience the full benefits of therapy:

    

– Enhanced Therapeutic Outcomes: Higher attendance rates are linked to greater improvements in emotional well-being, fewer hospital readmissions, and higher satisfaction with the therapeutic process.

    

– Reducing Dropout Risk: Attending sessions regularly helps clients stay engaged, reducing the likelihood of premature termination and fostering long-term success.

   

This benchmark is not about perfection but about prioritizing your well-being and maximizing the value of your therapeutic investment. Adhering to attendance policies reinforces the therapeutic alliance and provides a stable framework to support your progress.

     

When Attendance Drops Below 50%

      

When attendance rates fall below 50%, it can create challenges that may disrupt the therapeutic process. It’s important to approach this with understanding and curiosity rather than judgment.

   

– Understanding Engagement: Irregular attendance might signal barriers like time constraints, emotional overwhelm, or competing priorities. Exploring these together can illuminate solutions.

   

– Maintaining Continuity: Therapy relies on consistent touchpoints to sustain progress. Sporadic attendance can make it harder to build on past sessions or address emerging concerns effectively.

    

– Preventing Premature Termination: When attendance dwindles, the risk of leaving therapy altogether increases, often leaving unresolved challenges that may compound over time.

   

Clear cancellation and rescheduling policies can help minimize these disruptions. These policies are designed to provide structure while ensuring both the therapist and client can engage effectively and make the most of their time together.

    

Attendance Categories: A Compassionate Framework

      

To support a collaborative understanding, attendance rates are a ratio that identifies the number of times that clients did not show up for a session, late canceled with less than 24 hours of notice, canceled or rescheduled, alongside the number of sessions attended.  In other words, it’s the number of sessions that were attended as compared to the number that were scheduled.  If you have 1 no-show, 3 reschedules, and 2 late cancellations, and have only made it to two session, that’s 8 sessions scheduled with only 2 attended, which comes out to a 25% attendance rate. 

    

My approach here recognizes that perfection is not an expectation, and that things happen.  But it also recognizes that sustained patterns in behavior clearly demonstrate one’s commitment to the therapeutic process.  If you are showing up to 3 out of 4 scheduled session – that’s perfectly okay, you are still at the 75% rate where therapy begins to have an impact.  But if you are missing more than half of your scheduled session, that’s just not going to get you where you want to go – it tells me that you’re not prioritizing therapy.  When this happens, we either need to talk about prioritization or refer you out to another therapist. 

     

Here’s the way we map out the attendance rate percentages:

   

– Excellent (90% or better): Reflects a deep commitment to personal growth, setting the stage for optimal outcomes.

    

– Good (75%–89%): Indicates a solid investment in the process, aligning with meaningful progress.

 

– At-Risk (50%–74%): Suggests challenges that may need to be addressed together to restore consistency.

 

– Discharged (Below 50%): Clients who fall below 50% attendance will be discharged and/or referred to another therapist or practice.

     

This framework, coupled with respectful communication about cancellations and scheduling changes, reinforces the therapeutic relationship and highlights the shared effort required to achieve success.  It’s also good to remember the basics of our cancelations and no-show policies. 

        

Cancelations/Rescheduling (At Least 24 Hours’ Notice):  A cancellation or scheduling change request with at least 24 hours’ notice allows us tomanage your session without any charge. Providing sufficient notice ensures that we can adjust our schedules and, if possible, offer your time slot to another client who may be waiting for an appointment. This flexibility supports everyone in the practice and reflects the mutual respect that underpins the therapeutic relationship.

    

– Late Cancelations/Late Rescheduling (Less Than 24 Hours’ Notice): A late cancelation or schedule change request where notice is given with less than 24 hours of your scheduled session time.  These also incur a service fee. If you are using insurance through Oak River Wellness, this fee is $100. For clients paying out of pocket through Awakening Into Life, the fee is $65, equivalent to the full session rate.

   

– No Call/No Show: This is the terminology used when a client simply doesn’t show up and doesn’t communicate.  This incurs a service fee. If you are using insurance through Oak River Wellness, this fee is $100. For clients paying out of pocket through Awakening Into Life, the fee is $65, equivalent to the full session rate.

     

These fees are not intended to penalize but to encourage engagement, honor the therapeutic process, and sustain the integrity of the practice. Let’s explore the rationale behind this policy and its role in fostering a collaborative and respectful therapeutic environment.

   

Note: this policy does not apply to Medicaid clients, as Medicaid clients are not charged for late fees.  However, you must still maintain appropriate session attendance rates, or you will risk being discharged.

Late Cancelation Fees as a Motivator for Engagement

At the same time, the fee acknowledges that the therapist has prepared for the session and held that time specifically for you. For Medicaid clients, we understand the unique circumstances that often accompany accessing care through Medicaid. In alignment with our policies, we do not charge late fees for Medicaid clients. While the cancellation policy provides a framework, we also recognize that life is complex. How to Cancel or Reschedule

    

If you need to cancel or reschedule a session, you can do so by contacting us via phone, text, or email. We ask that you provide at least 24 hours’ notice whenever possible. This allows us to adjust our schedules and offer the time slot to another client.
